The Role:

We’re looking for a Vice President of Care Operations & Innovation to build and to lead Chamber’s care model — the operational core of how members experience our programs. You will design and implement high-performing care teams who make outbound calls, receive inbound support requests, and intervene on behalf of patients when needed — all designed to deliver better outcomes in a value-based cardiology model.

This role is hands-on — you’ll design the workflows, hire and develop care teams, implement enabling technology, and drive performance. You will thoughtfully incorporate AI tools and automation to reduce administrative burden and allow clinicians to work at the top of their license, ensuring care remains human-centered and clinically sound.

Key Responsibilities:

Design Systems That Scale

  • Develop workflows for triage, care coordination, transitions of care, education, and escalation.

  • Translate payer requirements and contracts into repeatable operational processes and accountability structures.

  • Build performance dashboards, QA programs, and capacity planning models.

  • Establish SOPs and operating standards that can scale nationally.

Use Technology as a Force Multiplier

  • Continuously simplify processes before adding headcount.

  • Introduce tools that reduce manual work and improve accuracy and consistency.

  • Partner with Product and Data teams to thoughtfully embed automation and AI where safe and appropriate.

  • Ensure technology enhances — not replaces — clinical judgment.

Build & Lead Exceptional Care Teams

  • Recruit, train, and scale nurses, care coordinators, and frontline leaders to proactively engage patients.

  • Drive excellence in communication, documentation, and value-based outcomes.

  • Develop clear productivity standards, career paths, and strong team culture.

  • Ensure teams operate at top-of-license with high engagement and retention.

Deliver Outcomes

  • Own SLAs, quality metrics, and cost-to-serve targets across markets.

  • Improve patient engagement, clinical outcomes, and operational efficiency.

  • Build a care model that scales without linear headcount growth.

What You’ll Achieve in Your First 90 Days

  • Stand up care teams with the right structure, capacity, and leadership coverage for live operations.

  • Launch operating standards for outreach, documentation, and quality.

  • Activate live care programs integrated with Chamber’s platform.

  • Implement initial tooling and process improvements that reduce administrative burden and improve productivity.

  • Lead an exec-level performance readout on engagement, capacity, and scalability readiness.

Requirements:

  • 10–15+ years in healthcare operations (population health, managed care, care coordination, or value-based care).

  • Demonstrated success building and scaling patient-facing operational teams across multiple markets.

  • Proven experience standing up programs from zero or early stage environments.

  • Strong operational rigor — fluent in staffing models, KPIs, SLAs, and continuous improvement.

  • Experience partnering cross-functionally with Product, Data, and Clinical leaders.

  • Comfortable adopting modern tools and automation to improve workflows responsibly.

  • Excellent communicator who influences executives, clinicians, and operators.

  • Passionate about transforming heart health for patients and providers.
